This Privacy Statement also does not address the privacy practices or policies of third parties with whom we may share your information as described in this policy, including Patient Ambassadors and medical professionals. Both our Patient Ambassadors and the medical professionals with whom we share your information, are independent from the BWB Program and are not otherwise affiliated with the BWB Program. Collection of Information

Personal Information

NuVasive may collect personal information about you when you voluntarily provide us with such information, including when you register to participate in the BWB Program or volunteer as a Patient Ambassador. This information may include your name; contact information (such as your email address, home address, phone number, ZIP code, or state of residence); and other information about you (such as your gender, date of birth, and whether you have back pain or back surgery).
NuVasive may also collect information from you when you complete a patient survey, use our website to identify a surgeon in your area, use a Better Way Back wearable to monitor your activity level, or communicate with us by any means.

Non-Personal Information

NuVasive may also collect non-personal information about your visit to our website.
